1. For Occupational Therapists to submit a bed authorization they should fill out the Bed Depot information Sheet below and send it via Fax or Email in addition to submitting the Authorization in the Blue Cross portal

2. The Occupational Therapist needs to submit their request into the Blue Cross portal.

3. To arrange for a bed pick up call 587.357.1511 and provide the following info

a. Client Name
b. Address where bed is located
c. Contact name and phone number

4. For clients wanting a bed through AADL they should contact their Health Care provider (either through the hospital or Home Care services) to arrange for a Home Care Bed
